HCCSC Wellness Program
June 1st 2022 – May 31st 2023 Incentive Year
The following significant changes will be implemented beginning on June 1, 2022
- Anyone actively enrolled in the HCCSC benefit plan by January 1, 2022 is eligible for the wellness incentive.
- The only requirement for your wellness incentive deposit in 2023 will be to complete a wellness physical and biometric screening with your preferred physician. (See the attached HCCSC Wellness Flyer.)
- Attached, you will also find a Physical Form. This form will be completed by you and your provider and sent to our data collection manager – Vitalincite. (See the attached form for instructions.)
- Because of this change, any physical you received after April 1st of 2022 will be counted.
- All physical forms are due by May 31st, 2023.
- After the form has been submitted and you receive e-mail confirmation of submission, you have met your ONLY requirement for the HSA incentive deposit.
- As always, the physical and basic biometric screening will be covered at no cost to members.
- Blood-draws will not be provided on-site at your location this incentive year.
- We understand the convenience of the on-site screenings, however, you should receive the screening as part of your annual exam.
- The incentive deposit is decreasing to $700.
- While we are aware this isn’t welcome news, the past several years the HCCSC Wellness incentive deposit has been significantly above average. Continually rising healthcare costs have forced us to decrease this payout.
- Wellness coaching will no longer be provided by Parkview.
- Desirae will still be available for individual consultation on an “as needed” basis through the remainder of the 2022 calendar year – all you need to do is reach out to her!
- At this time, access to near-site clinics (such as the Parkview clinic next to Kroger) are still an active benefit to members.
You will not have to report wellness points or track physical activity minutes.
Employee Assistance Program
HCCSC offers an Employee Assistance Program (EAP) through Parkview. The Parkview Employee Assistance Program can help you and your family members with a variety of difficult situations. The EAP counselors are knowledgeable in a variety of areas including:
- Alcohol and drug dependence
- Crisis intervention
- Divorce survival
- Emotional difficulties
- Family and marital problems
- Financial education
- Personal and work stress
- Troubled teenagers
Your confidential EAP services are provided to you and your family members who reside in the family’s home at no cost to you. You can contact Parkview EAP for an appointment by calling 260-266-8060 or toll free 1-800-721-8809.
Parkview EAP is also offering video counseling services to individuals 18 years of age or older and residing in the family’s home. Video counseling sessions are only available during regular business hours and must occur while the individual is within the State of Indiana.
